1. To enter, complete and return this Official Entry Form. All entries must be received by December 31, 2010. Mechanically reproduced entries will not be accepted. You may also obtain an official entry form by writing to: Paramount Builders Inc., 501 Central Dr., Virginia Beach, Va. 23454.
2. Grand Prize (1): Choose from either Preservation Replacement Windows for the entire home, or a complete house of Preservation Siding. Limit 15 double-hung or sliding windows/or 15 squares of siding. Prize does not include basement, garage or mobile home windows, or basement or mobile home siding (est. retail value $15,000).
First Prize (2): Winner's choice of a Bay, Bow or Garden Window (est. retail value $6,000 each).
Second Prize (1): Sliding Patio door (est. retail value $2,500). Third Prize (10): Double-Hung or Sliding Window (est. retail value $1,000 each). All prizes include installation. This sweepstakes consists of several separate thematic presentations. Winners will be selected in a random drawing from among all entries received from all presentations. Drawing will be conducted by Associated Materials Inc. who will choose an independent judging organization. Those decisions are final on all matters relating to this sweepstakes. Odds of winning are dependent upon the number of entries received. Winners will be notified by phone and mail. Limit one prize to an individual or household. Prizes are nontransferable and no substitutions are allowed. Sponsor assumes no responsibility or liability for damages, losses or injury resulting from acceptance or use of any prize. Taxes, if any, are the responsibility of the individual winners. Winners must take delivery of prize within 1 year of being notified by the distributor. In the event the winner has purchased a Preservation product that is all or part of their prize between 1/1/2010 and 12/31/2010, such prize winner may elect to receive the retail value of that prize in lieu of the product or products. Winners may be required to verify address or execute an affidavit of eligibility and release within 14 days of notification attempt or prize will be forfeited and an alternate winner selected. Entry constitutes permission to use winners' names and likenesses for promotional purposes without further compensation except where prohibited by law. No responsibility is assumed for lost, misdirected, illegible or late entries or mail.
3. Sweepstakes open to residents of the U.S. 18 years of age or older as of 1/1/2010, except employees and their families of Preservation and its dealers, its parent company, AMI, affiliates and subsidiaries. This offer is void in the states of Alaska, Arizona, California, Hawaii, Idaho, Montana, Nevada, New Mexico, North Dakota, Oregon, South Dakota, Utah, Washington, and Wyoming and wherever else prohibited, and is subject to all federal, state and local laws.
